AYSO Soccer Last Chance to Register

Hi Everyone,

Our last registration day is this weekend!  Saturday, April 28, 2012 from 9:00 am until 1:00 pm at Juan Cabrillo Elementary we will be holding our final in person registration for the Malibu AYSO 2012 Fall Soccer Season. Pre-register your children (must be 4 by 8/1/12) online at www.eayso.org (see the instructions below). Bring two signed copies of the pre-registration form along with your volunteer form and a check to registration. The registration fee for this registration is $200/child.  If a child did not participate in AYSO last year, a copy of the birth certificate or passport of that child is required for registration. For children playing in the Extra Program, this registration is for you too. There will not be a separate registration.
The pre-registration process is simple and the EAYSO program will walk you through the steps. Just in case, here are the instructions for pre-registering:

1. Go to www.eayso.org

2. If you are a first time user to this site, click First Time button. A screen where you may register your child, register as a volunteer or register as a Youth Volunteer will appear.

3. If you are a returning user and you have an EAYSO login, click on the Login button. You will receive a screen where you enter your email address and password.

4. If you are a first time user, the next screen will ask you to set up an account with a Login name and password.

5. A region selection screen appears next. “I want my child(ren) to play in region”, type in 759 for Malibu. Click NEXT.

6. The next screen will ask for the parent information. If you are a returning family, you will only need to update your information. The fields with the red * are mandatory fields to be completed Click NEXT.

7. The Child/Player Information page is next. Complete all of the fields with a red * next to it.

8. The next screen is the Agreement Page. Please take the time to read through the information and check the appropriate boxes.

9. The Volunteer Jobs screen is next. Indicate for which jobs you can volunteer. AYSO is an all volunteer organization. We need your help. Click NEXT to continue.

10. The final screen will give you the option to print your forms. You can export the printing to a pdf, word document etc. Please click the appropriate choice for printing. Printing without exporting the page will not allow the entire form to print properly. Please print 2 copies and sign them. Bring them with you to registration along with cash or a check made payable to Malibu AYSO. Sorry we do not accept credit or debit cards.

11. That's it!! Once you have pre-registered your children for the Malibu 2012 AYSO Fall Season and have registered yourself as a volunteer, you are finished.
